The windows file system is “lazy”, designed to minimize “write head” movement. Files are not always written in single continuous “blobs”. They are broken up into “blocks” of storage on the drive. The file system keeps track of how link these blocks together.

Initially, when the drive is empty, the unused space is continuous. When files are deleted, their space becomes available for writing new files. So the next time a file is written to the HD, it takes the first available free block. If the file is too big for a single block, it uses the next available block. Over time the free blocks become more scattered, making large files written in scattered blocks.

It is “fragmented”. The problem is that when a file is fragmented, the arm inside the HD that reads writes has to move around to find the separate blocks. This takes time, making reads and writes slower. This is done by reading a fragmented file, then writing it to another location often at the “end” of the HD. Later when there are large enough continuous blocks the files are moved from the temp location to the “front” of the drive.

Each defrag tool uses different rules on what files to select for defragging, and how to organize the re-written files. Yes, it is recommended to not defrag SSDs. Actually, some defrag tools are “smart” enough to refuse to defrag them.
